You're right - and the servers, switch ports, remote reboot ports, serial console ports, air conditioning, power, space and staffing are all paid off with your first $139/mo payment

The setup fees go into amortizing the cost of turn-up in exchange for a lower monthly fee. We give both options with our servers - so you really have your choice. It makes sense if you plan to stay with your host long-term. Why continue paying an additional $5-10/mo for the life of your server if you could pay $49 up-front. Whether or not a provider offers you a setup fee or no setup fee, it's factored into their pricing equation.
