In consideration of my engagement as a model, upon the terms herewith stated, and for good and valuable consideration herein acknowledged as received, I hereby grant to Peekaboo Internet Content Studios, hereinafter referred to as the ?Photographer?, his/her successors, licensees, legal representatives and assignees, those for whom Photographer is acting, those acting with his/her authority and permission (collectively, ?Authorized Parties?), the absolute right and unrestricted permission to copyright and use, re-use, publish, re-publish, license, assign, distribute, copy, edit, and alter photographic portraits or pictures, video, negatives, transparencies, digital information pertaining to them, in still, moving or video format, which I may be included, in whole or in part, or composite or distorted in character or form, without restrictions as to alterations, or reproduction thereof in colour or otherwise, in conjunction with my own or a fictitious name or names at Photographer?s sole discretion, made through any and all media or methods of transmission and/or distribution now or hereafter known, including but not limited to film, print, video, computer, World Wide Web, Internet Websites, Email, FTP, computer network, digital reproduction and distribution, for illustration, advertising, display, trade, promotion, art, editorial or any purpose whatsoever. I also consent to the use of any printed material used in conjunction therewith.


I hereby relinquish to the Photographer and Authorized Parties any right that I may have to examine or approve the completed product or products, printed copy, or any other materials or media that may be used in conjunction therewith, or the use to which it may be applied.


I further release, discharge, hold harmless and agree to defend the Photographer and Authorized Parties from any claims for remuneration associated with any form of damage, forseen or unforseen, associated with the proper commercial or artistic use of these images and publishing, licensing, and distribution thereof, including without limitation any claims for libel or invasion of privacy. I acknowledge and agree that this Release and agreement to discharge and hold harmless shall continue indefinitely, regardless of whether any image used causes me in the future to feel embarrassed, ashamed, degraded or otherwise injured in any manner., even, but not limited to as a result of any distortion, blurring or alteration, optical illusions, either intentionally or otherwise, that may occur or be reproduced in the taking, processing, distribution, publishing, posting of the finished images and products, or which may arise from any breach of any warranty, representation, covenant or agreement made by me. I also understand that injuries can occur and that I willingly take these risks and release the Photographer and Authorized Parties from any claims for remuneration associated with any form of injury, forseen or unforseen, during the taking of these photographs and or/video, or the travel to and from the shoot site.


I acknowledge that this release was willingly signed prior to the photo shoot. I hereby affirm that I am over the age of 18 and have the right to contract in my own name. I have read the above
authorization, release and agreement, prior to its execution, and I fully understand the contents thereof. This is the whole agreement, and supersedes any previous statements, agreements or discussion. The agreement shall be binding upon me and my heirs, legal representatives and assigns.
