One thing you missed out, but it provides perspective to everything else, is absolute cost. For example, adult webmasters expect custom blog designs to cost between $200 and $500. Mainstream designers typically charge between $1,500 and $5,000. Other design prices are similarly skewed.
A designer may continue to work for adult websites because he (or she) has established a reputation and doesn't want to start over, as it were. He may have learned how to cut enough corners to make adult prices worth his while, but still keep his clients happy. But realistically, many who have the ability to do the same work and be paid 5+ times as much for it, will follow the money: this includes operations which are effectively brokers for outsourced workers skilled enough to be used more profitably.
