Well, let's see... poor posture, walking improperly (leaning, weight shifted to one side, bad shoes), age, bad sleep (poor bed quality or just sleeping in strange positions), injury, improper heavy lifting.... and there's a lot more. All of these things can lead to having a spine out of alignment.
And there's very little a doctor can do about it besides suggesting better shoes, a better bed, sitting up straight and just being generally more conscious about your posture.
