I'm a programmer as well, and the only way I know of uniquely identifying remote machines with virtually no chance of mistakes (unless you're dealing with rather advanced hackers) is by exploiting clock skews.
Since I doubt that is what you are doing, there is a distinct possibility that your system is capable of making mistakes - unless, of course, you are dealing with someone who is not just a cheater, but a bad one as well. In that case, ID-ing becomes rather easy. (things like IP and cookies or other files would then easily give conclusive evidence)

From what I've seen in this thread, and looking at your site, the bit about "handling" isn't quite as clear. Of course, it could be that you make that clear in your members' area, or have made it clear in emails. And, of course, you have the right to ban anyone from your program, even with only a suspicion of cheating.
Looking at both bbwpixie and leroytrolley, though, it just seems like there are really two different people involved here.Even that could be untrue, naturally, but if it's not, it seems rather harsh that bbwpixie would lose the account for the actions of an employee.
