I'll generally hold the door open for any lady that's less than 3-5 paces behind me.
Guys, within the same radius, get the benefit of me giving the door another shove open as I pass through it.
If I notice it's an older lady, or older gentleman with a disability, I'll often extend my radius to 5-8 paces, or so, and am, naturally, a bit more patient in those cases.
And that's about it.
