M O D E L R E L E A S E A G R E E M E N T
· I hereby give the Photographer, his representatives, assigns and licensees, those acting upon his authority, those for whom he is acting and those acting with his permission and their respective agents and employees, and any publisher, who may hereafter publish and exhibit the Photographs (hereinafter collectively referred to as the “Users”), the absolute right and permission to copyright, use, reuse, publish, edit, alter or modify, sell, resell, exhibit, display, print, reprint, promote, display in magazines, books or any other form of media including the Internet, or for any other purposes, the Photographs heretofore taken, or taken this day, or hereafter taken by the Photographer and for which I have acted as a model, including without limitation, reproductions thereof of which I may be included in whole or in part. The failure of the Photographer to pay for such use shall be not deemed as a failure of consideration by any other Users. I understand that the term “Photograph” used herein encompasses all photography or videography produced or provided to Photographer for this and only this project.
· I hereby waive any right to inspect or approve the Photographs or the editorial or advertising copy or printed matter that may be used by the Users in conjunction therewith, and further waive any claim that I may have with the respect to the eventual use to which it may be applied. Such photographs may be used in the sole discretion of the Users, with or without my name or with a fictitious name, and with fictitious or accurate biographical material, alone or in conjunction with any other material of any kind or nature. I hereby release, discharge and agree to save harmless the Users and their licensees and assigns from and against any and all liability in connection with the use of such Photographs. Without limiting the foregoing, the Photographs may be used in any media form, such as magazines, video, Internet etc. which may contain sexual material.
· I hereby represent that I am eighteen (18) years of age or older, and that I was eighteen (18) years of age or older at the time the Photographs were taken. I further represent that I provided two (2) valid and correct identification documents to the Photographer and that a true and accurate photocopy or photograph of each identification document is attached to this Release Agreement.
· I hereby state that this photoshoot was done in a professional manor.
· I further represent that I have read the Release Agreement prior to signing it. I have not been induced to sign the same by any representation or statement made by the Photographer or his agents, employees or anyone acting on his behalf. Furthermore, by signing below I agree to and understand this agreement.
