I don't have the facts but I think the problem was with the launch 360s and they have since improved the design. MS extended the warranty from 1 to 3 years and did everything possible to rectify the flawed design. The time they took to fix and return the 360s was less than two weeks and they lost millions because of it.
Generally, the 360 community was understanding and satisfied with MS actions.
It was unfortunate and it seems that the newest generation of 360s are much more reliable and now you pretty much have to play it to death to kill it
