At first I was appalled that he was so cavalier about it, about keeping on with his smoking habit even though he's facing his own demise. But after thinking about it I can only surmise that it's one of those things where you only have so many simple pleasures left in your life, and for him smoking is one of them. Giving up the habit at the point he's at probably wouldn't do much towards "saving his life" anyway, he's been at it too long.
So I can sort of see why he would choose not to give it up, in that light. (see what I did there? "light?" :D )
