Reseller authroization from the primary producer is important as well, but that's different... the 2257 affidavit is the statement signed by the producer swearing that he checked the performer's ID and that the performer was infact over 18 at the time of production. This should be a separtate one page form, but a lot of companies include this statement in the model release itself. Either way is fine for us as long as there is paperwork stating that producer signed off that he checked ID's and the performers were over 18 as of the production date.
