We have been successful in getting our material removed in the past by simply sending a DMCA notice to the hosting company by email. The email below works for us every time. Feel free to use it! Be sure to replace everything in BOLD with the proper information. You will also need to find out where the site is hosted. (Ping it, then enter the IP address into Arin.net to get the host provider info).
