What separates the simulated rape in a mainstream movie from simulated rape in porn is "prurient interest" -- the first is a movie that tells a story, the second is something meant to bring on a sexual response. It all comes down to obscenity laws, which VISA is (rightfully) trying to stay within. I'm no apologist for VISA, but they gotta keep their biz in line.
Argue the rights/wrongs/morality/etc. of it all you want, but that's just the way it is.
