Obviously things have come a long way since then and athletes are expected to hide their natural reactions and bite their lip, which is unfortunate for her.

The foot fault call and how often it is or isn't called has nothing to do with it. End result would be the same if she got a called double-fault and reacted that way. She didn't lose because of the foot fault, she lost because she committed unsportsmanlike conduct after the call.
As for McEnroe, he very well may have reacted the same way and would've gotten the unsportsmanlike conduct call as well. And he would've looked like as big a douchebag as Serena looked.
With your own match, unless I'm mistaken, there isn't a damn thing you can do about the other team. A sanctioned official would have to be strolling by and they'd have to make that call against the other team. I could easily be wrong on that, though. I don't play but my wife does.
Come to think of it, McEnroe pretty much always looked like a douchebag.
