I had a triple entry.
After 2 months I went to the local immigration office and got another 30 days the fee is 1900 THB and another 100 THB because I did not have a photo, quick and painless process except for the 70 miles each way on motobike.
You can get the extension a few weeks before your 60 day visa expires but if you visa expires January 17th then the last day you are allowed in the country Jan 16th and its also the last day to extend the visa also your visa have an Enter By date you must enter the day before this date to activate your second 60 days visa.
You don't have to leave for 24 hours just head over the border and back 30 minutes later that what I did on one of the visa runs, there are different companies that offer visa runs to Cambodia and its fairly painless.
