I forget the exact % but iPhone users consume 60% more inet than any other phone users. So as the # of subscribers climb even the largest network will start getting taxed and before you know it that one-fee-for-everything plan will either start having the same issues as AT&T or they will switch to a pay-for-service model which is what AT&T is considering.
Btw, if AT&T switched to that you would see their level of service improve dramatically as well because of current over consumption.
