Then their host.
Then their registrar.
If they have a processor, like rapidshare does PP, you start CC'g their legal department.
If they are in the U.S., you also include CC's to the attorney general of their state in correspondence. You will find that the higher up the food chain you go, the faster they comply and they will actively police your content off their sites from then on.
Even if they are out of the U.S., most have an achillies heel. U.S. host, or registrar, or payment processor.
One way or another you can get to 97% of them to comply.
