I think the only person responsible for Michael Jackson looking bizzare was Michael Jackson-
I think this is also a very fair looking pic of him. He has pushed his body to limits that are not found in nature and that is where the problems lie. He has bleached his skin whiter than the lightest caucasian, and has altered his facial features to the point they look inhuman.
He probably had a combination of body dysmorphic disorder and little richard syndrome.
Body dysmorphic disorder is when you don't see yourself the same way the world does. In it's lightest form you can see the manifestation in people who can't stop bleaching their teeth because they don't think they look white enough or people who bleach the living sht out of their hair because they don't think it looks blonde enough, much like MJ did with his skin. In harsher cases it's the culprit behind anorexic and bulemic girls who starve themselves to the point of death or hospitalization because they don't think they're thin enough or people who keep going in to the plastic surgeon to re-alter already adjusted features, like maybe they don't think their nose looks small enough, etc.
Little Richard Syndrome is essentially when you surround yourself with too many yes men and bannish anyone from your entourage who expresses an alternate opinion to the point you have no one left in your life to tell you you look a mess and are acting a fool. I don't think anyone could argue that this damaged him on not just one but many levels.

The Michael on the left is how I choose to remember him. He'd already begun to alter himself, but at least it was somewhat tasteful. The Michael on the right in comparison, to me, looks a little disturbing. Like a bleached out carol channing on anabolic steroids.
