Here is the reason:
Money.
It really is that simple. Any movie that has real sex in it will never get a mass release. If a movie has too much "movie sex" in it then it gets an NC-17 rating and will never get a wide release. They could make these movies and release them on DVD and pay per view, but then you have the issue of marketing it. It will be hard for it to find an audience if it doesn't have a known actor in it. If you want to put a known actor in it then it is going to cost you money.
In the end most erotic style movies don't make much in the way of profit. The movie studies want as close to a sure bet as they can get so they stick to things that they know will draw audiences and that they can get on 2,000 screens with a release.
