There was an AIDS conference in Spain last year, where they concluded that getting AIDS from oral sex is pretty rare.

[TITLE:] Risk of HIV infection attributable to oral sex among MSM and in the MSM population: the HIV Oral Transmission (HOT) Study

Results: Of 10,283 ATS clients, 243 (2.3%) participated after 3 eligibility screenings. 239 (98.3%) were MSM (median age=39 yrs) who had performed ROI on a median of 3 partners; 98% without condoms. 35% were exposed to ejaculate in their mouth; 28% performed ROI on a known HIV+ partner. Of those, 81% did not use a condom and 39% reported swallowing ejaculate. No (0) recently acquired HIV infections were detected, and estimated probability of orally acquired HIV infection was 0 (95% CI: 0, 1.5). Using our previous published RR=1.01 per ROI partner and population prevalence of ROI, PAR% increases from 0.18% to 0.56% as exposure to ROI partners and RR increases.
Conclusions: A very small proportion of persons seeking HIV testing engage exclusively in ROI. The very low probability of HIV infection in this sample confirms that orally acquired HIV infection is a rare event. The PAR% for HIV due to ROI is 0.56% or less. Since a smaller fraction (35%) of those who practice ROI have exposure to semen in the mouth, the actual PAR% is likely to be lower.
