Somebody must have a couple sites that they think have a good custom form that increase the overall conversions... it's only the last step to an impulse purchase decision and would think that the form itself would likely be the last and final selling point to an online purchase.

I don't think it's the MAIN factor, but it's definitely a contributing factor to better ratios.
