1. Rent an office in a building with lots of other offices - having to get out of the house and seeing other people rushing about helped me

2. Spend more money, buy an expensive car, remodel your kitchen or something like that. The more i spend the more motivated i get. Sitting back and watching the bank account grow made me lazy.

3. Pretend you had to look for a 9-to-5 job. Look at the job classifieds. Go on a few job interviews. The thought of having to work for someone else makes me wanna puke and lights a fire under my ass. You need to remind yourself how good you have it.

For the Euro crowd renting an office is a waste of time, as you usually work more towards US hours, so you find yourself in an empty building with the cleaners.

I take the laptop up into the mountains for a few days in a cabin, or last year I rented a villa for the summer, worked 6 hours a day instead of the usual 12+ and enjoyed the beach. A complete holiday does my head in a bit, but reduced hours does wonders for productivity.
