I've worked with it since 72 in various settings and as was already stated, you have to have a base desire to want to change whatever habit you're wanting to break.. or 'replace'
Yes it can work.. no, psychiatrists don't want to admit it because they deal with long term, 'see you next week' therapy... that's their bread & butter, where hypnosis is more short term result oriented.
If you try not to dramatize what hypnosis actually is, you can better understand why it can work..
