I think it's due more to inactivity than how much people eat or even what people eat. If Michael Phelps can eat several thousands of calories per meal and stay in shape, that should say something. I've seen it in my own life...I was much more active when I was younger, and I stayed in reasonable shape despite having fast food pretty much every day, sometimes twice a day, and other junk food. Today I eat LESS than I did then but gain weight from it - because I'm on the computer all the fucking time. Even if I get exercise in every now and then, I'm sedentary the rest of the time so metabolism stays low.
It pretty much really is as simple as what gets exerted vs. what comes in, with the X factor being metabolism being higher for more active people, and in this computer/internet age, people are just on it all the fucking time and don't get enough activity to maintain reasonable metabolism and calorie burn. Combined with thinking a short-term diet is the answer and going on it only to binge off of it and be worse off, most people are pretty well fucked and have to resort to surgeries that didn't even exist a few short decades ago.
San Diego has always been one of the more healthier cities...not because the food here is healthier or even that people eat less of it, but because the awesome climate here attracts the more active people.
