a carb load every 3 days would help offset the issues but a pure ketogenic diet isn't healthy long term for several reasons.

1. it's a huge stress on the liver
2. the risk of going ketoacidotic (spilling ketones into the urine which is the primary issue for diabetics)
3. the imbalance of nutrients, a ketogenic diets requires supplementation with vitamins and nutrients missing in the diet, ketogenic diet means extremely little to no fruits, vegetables, calcium rich foods, etc.

it's a controversial diet that is used for weight loss but originally designed to treat epilepsy in children.

i don't see the reason to maintain it long term in light of the issues and when a solid, balanced diet is perfectly fine.
