Yes, no peeing in the crate! With Daisy we had to stuff an entire banker's box upside down inside her crate until she was 6 months old to fill up the extra space!

Sure, I see no reason not to carry him until you're outside. That will reinforce the 'this isn't appropriate' feeling. But realize that accidents will definitely happen. The key is consistency! For our pup's first month or so it went like this and we had surprisingly few accidents:

pee: 6am (or when doggy wakes up)
back to bed with NO PLAYING OR CUDDLING until your regular wakeup time
pee: 8am (or when YOU wake up)
breakfast
pee within 10 minutes of finishing breakfast
exercise (at least 15 mins, increasing with age)
pee within 60 mins of finishing breakfast
pee every hour after that
lunch (puppies metabolize very quickly and have small tummies. three meals a day until they're a few months old is a great idea)
pee within 15 mins of finishing lunch
pee within 60 mins of finishing lunch
pee every hour
dinner
pee within 15 mins of finishing dinner
exercise (increasing with age)
pee within 60 mins of dinner
pee every hour
pee right before bed
probably a pee at 2-4 am

If he has to pee in the middle of the night, there should be no extra playing or talking. Just quietly put in the pee spot, say the command 'go pee' or whatever, and lots of praise when done. Then straight back to the crate/bed. This should not be a ruse for playtime.

Good luck!!

Like I mentioned before, at one month of age, a pee every hour. At two months, every two hours. By five months he should be able to hold his bladder for five hours. The more consistent you are, the fewer accidents you'll have!
