I would say write up the fields you want, then have some cheap program at a site like odesk.com write you a simple PHP / mysql script with search. It shouldn't cost you more than $20 or $30

Basically ask for things like

name
email
fetishes
gifts purchased
likes
dislikes
dog's name

whatever - fields you want to keep up with.

Then you can have the list sorted by name alphabetically or you can search for a custom by name or email. You can click the name and EDIT the entry or you can click ADD NEW entry and simply fill out a form to add a new client to track.
