I agree, it is a risk that they knowingly assume.
What sucks though, especially with HIV, is that the burden of treatment usually falls on taxpayers. What's anti-retorival drugs costing these days? $4-6k a month, I think. Sad part is a lot of talent do not have health insurance, and getting health insurance with HIV is nearly impossible, so it falls onto Medicaid.
If you get shot working at 7-11, or burn your hand on the grill at Wendy's, the employer and their insurer are responsible for all medical costs.
