When I was 19 I was "nominated" by the company president to have to tell an employee there was a BO issue. I was young and intimidated, so the best thing I could come up with was printing out the page in the employee handbook that mentioned hygene and highlighting the parts I felt applied to her. I ended up for the most part just handing her the paper to read herself.
It didn't go over well. Her boyfriend, who also worked there, screamed at me for hurting her feelings and said she couldn't bathe regularly because she had a heart condition (WTF?!). The problem was short lived though as they got in a fist fight less than a week later.
If you have HR that's the person who needs to deliver the news, if not the best way to deal with it is to just bite the bullet and have the conversation in the most gentle and polite way possible. If an employee has a BO problem it's unfair to your other employees to let it go and not address it.
