Long form = you, the human being, were born in a hospital by a doctor who signed said form as well as your parent/s.. You don't have to get this form from the hospital. A certified copy can be obtained from vital records.
Short form = A "person" who is living who needs proof he/she is who he/she is claiming to be. IE. A person who is claiming to be say, Barack Obama, can have a short form. Not a long form. Because the "name" Barack Obama was born in the US. A short form "person" (look up the legal definition of person and it also means corporation) represents a name. Let's say you were for some odd reason put into a witness protection program. YOu would receive a short form.
