Have they tried looking at other payment processors like Verotel (I know they have some form of affiliate section, but i'm not sure how well it would work or even how it works, but I think it will handle the payouts like CCBill does). I'm not sure about the Visa thing though as it might be Visa's rules rather than the payment processors, unless they are just being extra cautious.
As for the design and layout of the site, they may need to find someone to help them build in the final features and integrate the payment processing.
