Not sure what to say that might be helpful other than you have my sympathy -
can you block the people on your personal facebook and twitter? I have separate ones for personal versus modeling, maybe encourage the fans to add your modeling ones instead, play dumb when people who shouldn't know your real name do?
Really, I'm sorry it's happening to you, there is generally and understanding not to out people's personal info if they don't want it out, at least that's my understanding.
