I believe it's both. For some, like me, the mental / psychological is more prevalent. An addictive personality, some call it. Underlying my actions are my thoughts, the mental / psychological thing you mentioned. Underlying my thoughts is a deeper level, what some call the spiritual level. Since I have the addictive personality / spirit, I'll smoke ridiculous amounts of weed, drink to excess, whatever.
There is also a physical on/off switch, a difference in the way some people's bodies process alcohol specifically, and it's genetic. Texas A&M university did some research on that. For those with the physical allergy, a couple of drinks creates a physical chain reaction it seems, having to do with the acetone stage of metabolism. This phenomenon of craving is occurs only in those with the physical difference, the biological alcoholic.
This craving chain reaction is, I think, why an alcoholic's behavior can be so mysterious to a non-alcoholic. "Can't you learn to just drink like a gentleman." No, no more than a diabetic can learn to eat candy. The non-alcoholic never experiences the chain reaction of craving and so does not understand it.
