www.createspace.com which is owned by amazon. I don't know what the fees are but they sell e books, they have a wizard where you can make a book. You can design or make a cover from a template then upload the word file. You then can sell it in their marketplace and they print it out per order. I think you can sell dvds as well. I know a few people who have used it for books they could not get published and they just deduct whatever fee from the orders.
