Make sure to select the following:

When done it'll let you submit a list of links for removal, 1000 links max in one day. Once submitted, you can follow your this and all concequent DMCA take down requests with Google at their DMCA Dashboard:

It typically takes 1-2 weeks to process a takedown request - they do not reply right away, so just wait. When you get a reply, it is usually either a takedown of links confirmation, or they're asking for additional info - they have a template that basically says "who the fuck you are and why do you submit links to us for removal". To avoid that you can include a good description of yourself in your takedown request, who you are and who you represent, and an explanation of why those links you're sending them are infringing. It'll save you alot of time - but not necessary, because Google employees are surprisingly stupid. Make a template for your reply because each new guy handling your new DMCAs will be asking the same questions about you, using the same templated message. If that happens just send them your templated answer, and also include it in the comments box with each DMCA, because there's a chance they'll notice it and will not answer the same question again.

Also make sure to create an account that matches your business entity, and has your business e-mail in the profile. Submit your DMCAs from that account, not from the one you're using for personal things.

Send links to content pages only, they do not accept blanket DMCAs to remove entire sites. Make sure to exclude pages such as fag.html or help.html that do not feature any infringing content. Google will only remove those links where your infringing content is being "shared" directly - i.e. direct links to blog and forum posts, torrent downloads etc.
