How? They keep changing URL and payment accounts. It's a cat and mouse game, but the market is so big, that it's worth it. The biggest problem is the payment accounts (people need to be able to pay with local bank cards and other local payment methods) and of course the language barrier and infrastructure (hiding your sites from the government/automatically keep moving urls etc) are hard to overcome as well. You need the right contacts and inside people that are willing to take the risk for you in exchange for a percentage. It might sound hard, but there are always people willing to take big risks if a lot of money is involved.
I think the same could apply for porn. There are twice as many people in China than the US and Europe combined and there is hardly any competition. Of course there are a lot of poor people in China, but there are also millions and millions of people that have the money. With the right contacts there are many millions to be made on the Chinese porn market.
