Payserve has those output options as CSV plus more.
The only thing that I've had problems with, though, is for some galleries, there isn't a thumb once in a while. To filter those out, go to
http://www.textmechanic.com/Remove-L...ontaining.html and paste the output with .jpg in 'search lines for' and click 'not containing'. Problem solved
