I find chargebacks happen most when the customer gets overdraft fees as a result of the charge. All of a sudden they are faced with over $240 in overdraft fees and there only option is to pretend like their card got stolen or they didn't authorize it. Most of the time the bank will call the number that appears on the credit card statement and if there is a live operator available sometimes the chargeback can be avoided by issuing a refund right there on the spot.
