Girls gone wild is trademarked and Google knows it, they won't let you have it. I was the first one to have my chops busted by the company that owns the trademark, Google got a C&D letter from the owners of the term, my rep contacted me about it, and we decided to just drop the word - of course.

When you find competitors getting ads for words you are not getting by with, email your adwords editor about it, they're quite helpful. Often it is because the adgroup was never reviewed.

You're right about Premium, it is much more "lax", BUT if you don't already have an account with Premium, you can't buy into it now, the Premium program will end as of the end of this year, adwords will now encompass both the 2 top ad spots and the side bar as we know it. None of the details are available, but everyone will have a shot at the top spots, it will be PPC, not CPM.
