Necessity. Everyone thinks they need a phone and they can already shit in the street and have been since the beginning of time, so why bother making more toilets?

Developing countries are not easy to pin down to one reason why something is done, and if there are cultural issues involved any change whatsoever can take decades. Most of these places are incredibly complex with their various religions and cultural beliefs, not to mention just not knowing any better due to lack of education. It often defies our version of logic and everything we as westerners have come to know. But many of them say the same thing about us.

This is one of my favorite images. It's funny, but it is a reality check at the same time. I know you've seen it before, but really soak the message in. We can't fathom the horrible conditions they live in, yet they can't believe we shit in clean water. It makes perfect sense for us, but to them it defies all logic. It's not easy to change something like that.
