Epoch has a mishap like that a few months ago, got a check on a regular piece of paper lol. They sent out a replacement right away but it was amusing none the less.
Also as far as Nats and Quickbooks goes it's easy peasy to have it set so the affiliate id shows up with the payment details on the stub. That said the issue is if an affiliate uses the same ID for every sponsor, or companies that own multiple programs use the same company checks to pay out for all programs.
