That's what I'm referring to, the "convenient store" debit cards. They are all associated with some bank account. Money isn't tied to a specific account number on a card, otherwise if you lost it, like a gift card, you are fucked.

Edit: let me rephrase that... The debit card account number is the gateway between the account where the funds are and the debit card itself.

Some non visa/mc cards can limit spending at an account level, like a gift card from Target. In the back end it says "account #12345 has a max limit of $500," however that's not the way Visa/MC debit cards work - they must be tied to a specific cell account. A friend of mine got into the prepaid debit card biz, that's where I learned some of this info from.

They are merely funding the card through a refund method right there. Banks in the US don't so that because they like to charge you fees for shit like wire transfers.

Most of your questions towards the US can simply be answered by "is there money to be made doing this?" If the answer is "no" then no one will do it.
