My guess is that they were set up to process cardholder present transactions for tangible goods over the telephone (for some reason) but they used it for cardholder not present transactions for phone services?

Without actually calling the bank again as it has been a long time ago now, I believe that was the case. This bank (HMS) works very closely with us and has been processing adult for over 15 years. We have hundreds of merchants with solid accounts there and we continue to setup and process for new accounts weekly.
