Here is the nuts and bolts of it if it hasn't been explained to you yet. (I didn't read the whole thread so I apologize if it is a reapt)
Send them another DMCA for your content at the same time send a DMCA to the webhost which you have identified. Note that your previous requests have been ignored. That host is a reseller so try and find their up line provider, and send them a DMCA as well. Send the domain register for a host a DMCA as well. After the requisite time has passed ignore the tube altogether and go after the hosting companies and domain register they won't be able to ignore the requests any longer.
