This is very very true and you learn this through bitter experience.
I have gotten to the point where I will write out a long narrative, provide example sites and what I like about them, I will cut and paste layouts into photoshop to make graphics of layouts and how I want them.
Needless to say, it takes a lot more prep time and you could spend a whole morning on it. However, I have found that you will get better results. Most of the stuff delivered will be 75-90% of what I wanted on target, with some minor changes to be made.
You have to think about how you want your project to flow.
