100% Home office since 2007.
I have a dedicated office in the house just for my business. My wife and kids understand that once the door is closed I am "at work" ... My productivity has been FAR better at the home office where I can think, plot and be free from the mindless office banter that just kills a few hours in a day.
My team has set times that we have conference calls to talk biz. We are very organized with our agenda and action plans.... This alone has saved a ton of the impromptu calls that also kill your "primetime" as I call the working hours.
Moving to home offices has made my team FAR happier. They can at times get the personal things done ( car to the shop for an oil change, pickup kids, have a decent lunch, etc ) and also perform at a much higher rate than when we were all in the office.
Once a month: We all meet up for lunch and talk live. At this point: Its good to see each other in person. This gives some human interaction outside my wife and son ... plus I also hang out with my friends every so often ( which is not all that much as were all married with kids now ) ... mostly I enjoy just being with my family at home.
My advice to be as productive as possible from home: Make your work space just that - your WORK space. Set time limits to be there .. In my case: From 730am to 6pm I am in my office and post 6pm I roll downstairs to see my family, have dinner and lounge about in my living room/outside if its nice out. The most I have at night near me is an iPad for non-work surfing and my phone....
