When the wife and I ran our company, we ran charges ourselves (never let the girls have cc info...), and used a virtual secretary for our phone line. We found the best to be Grasshopper (which was GotVMail when we started). We just set up a shit ton of extensions, and gave each character an extension on our line. Initially, with only 1 site and about 20 girls, our phone bill was low. As we expanded, our monthly toll free bill was upwards of $800. Of course we were grossing a hell of a lot more than that.
So you can set up with low costs and without buying expensive software, unless you wanted automated call length tracking and billing. The hardest part now is the marketing, inmho. There's still big money to be made, as you're selling the personal interaction (like cam shows). It's just a matter of getting it out there the right way.
