Here is a story from the time I was in the army:

We had a regular exercise and at the latter stages our instructor made us to simulate carrying a wounded man. We made a stretcher and carried the "wounded man" in turns, in some fucking thick woods/ swamp. Some of us gave their guns (assault rifles) and some other gear to others while carrying the wounded. Well.. one guy gave me his gun and at some point as I tried to get a better grip from the gun (I had one gun in each hand), I accidentally pulled the trigger and the result was a good burst while the gun barrel was pointing to my foot. Well, there was only ammo with wooden bullets and the bullets splintered on a metallic device and threw the pieces on the side, so no harm to me (or to others). In this situation I would have appreciated that the safety would have been on, and I should have checked that it is on. I don't even know why the safety was not on, because we didn't have any threat of "enemies" at that point, as we were merely returning to base from long battle exercise, when the instructor decided that this ain't over yet.
