Just for him to enter and leave the Kremlin requires a Lot of police and it's a huge ordeal - the cars do over 100mph through the city with an army of police running ahead and stopping traffic, blocking roads, security vehicles, medical etc etc. I used to live right off of Kutuzovsky Prospekt which ended a couple blocks at the main entrance to the Kremlin. My point is that I've seen it countless times and for the prime minister or president to move around, it's a huge thing and doesn't go unnoticed.
And as said, every time a leader is out of the spotlight for a week, people start speculating they are dead as if that's the first, most reasonable conclusion
