Websites with authority rank on the top (ie: xxx tubes). The only way to increase authority is links. You can have the best site in the World, and you will never rank with no links, unless you update it 3 or 5 times a day. This is not a complete explanation, and a lot of people wont agree, but this is, more or less, what it is. Small groups of high authority websites take most of the traffic in every niche (at least adult, I am not familiar with anything else, but it it most likely the same).
There is the Trust Rank thing too (I am not sure about it, I just read about it today) - hand picked websites with the most trust from Google.com (could be PornHub.com, xVideos.com, and so on).
