The government pays for drugs included into the
mandatory list - use Google translate.
For example, my father is a former boxer and he is suffering of Parkinson disease. He's receiving his meds for free. Even a wheelchair was provided by the government. Course you are right, nothing is really free in this life. I do pay taxes as anybody else: 6% from my income. But if you have no job, those free meds and clinics are still free for you.
P.S. Here in Russia we have both free and paid medicine. So if you don't like free clinics, you can get your treatment in the paid ones. At least, everybody here has a choice.
